Self-declaration form for the use of the Actv discounted ticket price for school groups ​ ​ 

The following form must be completed by all school groups using the Actv discounted ticket price for student groups for Actv public transport. A list of the names of the students in the student group who will benefit from the discounted ticket price must be attached in PDF format on the letterhead of the school/educational institution organising the trip.
